Rudolph Jon Polivka

Mr. Rudolph Jon Polivka, 91, passed away Monday, April 20, 2009 at Forsyth Medical Center. He was born November 22, 1917 in Berwyn, Illinois to the late Rudolph and Marie Kuncel Polivka. Mr. Polivka retired from Western Electric with 45 years of service and was a U.S. Army veteran during WWII. He was a charter member and Past President of Pinebrook Country Club and was a mason for 52 years. Mr. Polivka was a lifelong Chicago Cubs fan and a long time supporter of Virginia Tech athletics. He was a founding member of the Winston-Salem Minor League Booster Club and a member of the Telephone Pioneers of America. He is survived by his wife Betty Polivka of the home; two sons, Alan Polivka and wife Jane of Winston-Salem and Mark Polivka of Lewisville; two grandchildren, Matthew and Kristen Polivka; three sisters, Mary Maruna, Violet Kopecky, and Georgiana Whitaker; also surviving is a nephew and two nieces. A celebration of life service will be held 1:00 PM Saturday, April 25, 2009 at Hayworth-Miller Silas Creek Chapel with Rev. David Merritt officiating. The family will receive friends following the service.

In lieu of flowers, memorials may be made to the Brenner Children's Hospital, Medical Center Blvd., Winston-Salem, NC 27157.
