Stokesdale - Denise Marie Smith, 52, a loving mother, passed away Wednesday, January 04, 2012 at her home. She was born August 7, 1959 in IL. Denise was employed by Forsyth Medical Center as a dietitian. She was preceded in death by her mother, Dolores Kowalczyk Pachura. Surviving are her husband, Rex Smith of the home; her two daughters, Alyssa and Danielle Smith both of the home; father, Dennis Pachura, and step-mother, Rose of Sun Lakes, AZ; brother, David Pachura of Fort Worth, TX; sister, Janet Lakey of Hurst, TX; step-daughter, Daphne Bice; step-granddaughter, Courtney Bice; and many loving family and friends A Funeral Service will be conducted at Saturday, January 7, 2012 at Hayworth Miller Kernersville Chapel with Pastor Jerry Walker officiating.
