WINSTON-SALEM-Mr. Charles ""Chuck"" John Kotofsky, Sr., 65, of Winston-Salem, passed away on Friday, March 21, 2014 at Duke University Hospital. He was born on December 24, 1948 in Cleveland, Ohio to Robert Charles Smith and Virginia Marie Mares Smith. Chuck served his country in the United States Army and participated in the Vietnam War. Chuck was always known as a honest, hardworking man all of his life. He was a beloved husband, father, grandfather and friend to everyone he came in contact with. He will be missed dearly by all who knew him but now he is with his Lord and Savior, whom he loved above all. Mr. Kotofsky was preceded in death by his father. Mr. Kotofsky is survived by his wife, Shirley Raker Kotofsky of the home; two daughters, Sharon K. Smith (Shawn) of Wallburg, NC and Lisa K. Gillenwater (Harley) of Winston-Salem; son, Charles J. Kotofsky, II (Amy) of Winston-Salem; grandchildren, Skyler Smith, Sterling Smith, Savannah Smith, Kayleigh Kotofsky, Charlie Kotofsky and Cameron Kotofsky; mother, Virginia Pelham of New Mexico; and three brothers, Joseph Kotofsky (Pamela) of Tennessee, Greg Kotofsky (Lucy) of Florida and Dwight Kotofsky of Washington State. The family will receive friends from 6:00 PM until 8:00 PM on Monday, March 24, 2014 at Hayworth-Miller Silas Creek Chapel.
