KERNERSVILLE-Mrs. Annie Blanche Hayworth Smith, who was blessed to live 100 years, passed away Monday, February 14, 2011 at Forsyth Medical Center after a brief illness. She was born May 6, 1910 in Guilford County to the late Alfred Darius and Mary Ethel Hammer Hayworth. In addition to her parents she was preceded in death by her husband of 70 years, Curtis Ray Smith; a son, Van Dale Smith; daughter, Peggy Ann S. Conrad; great-granddaughter, Christina Leigh Swaim; daughter-in-law, Jeanie H. Smith; two brothers, Belvin L. Hayworth and David A. Hayworth; three sisters, Ruth H. Hedgecock, Maude Victoria Hayworth, and Norma H. Squires. Surviving are three daughters, Jean S. Clodfelter and husband, Norman, Linda S. Murray and husband, Craig all of Winston Salem, Vickie S. Rothrock and husband, David of Kingsport, TN; three sons, Curtis Wayne Smith, Derris H. Smith and wife, Rachel, Bobby Earl Smith and wife, Jane all of Winston Salem; 18 grandchildren; 23 great-grandchildren; 3 great-great-grandchildren; two sisters, Mary Helen Craver of High Point, Madge H. Carrico of VA; daughter-in-law, Nancy P. Smith; and son-in-law, Robert A. Conrad. A Funeral Service will be conducted at 11:00 am Thursday, February 17, 2011 at Hayworth Miller Kernersville Chapel with Elder E.W. Hooven and Rev. Joe McWethy officiating. Burial will follow in Oaklawn Memorial Gardens. The family will receive friends from 7-9:00 pm Wednesday, February 16, 2011 at the funeral home.
