Amanda Leigh Cook

WINSTON-SALEM-- Ms. Amanda Leigh Cook, 38, of Winston-Salem, went home to be with her Lord and Savior on March 3, 2014. She was born on July 1, 1975 in Forsyth County to Roger Lee Cook, Sr. and Pattie Smith Cook. Ms. Cook was preceded in death along with her father by her daughter, Chelsea Marie Cook; brother, Roger Lee Cook, Jr.; and her maternal grandmother, Billie Burrow. Ms. Cook is survived by two daughters, Hannah Gabrielle Cook of Sandy Ridge, NC and Savannah L. Smith of Winston-Salem; son, Paxton Roger Smith of Winston-Salem; mother, Pattie Smith Cook of Winston-Salem; two uncles, Ron Burrow (Lisa) of Winston-Salem and Fred Smith (Rena) of Kernersville, NC; father of Savannah and Paxton, Michael Smith and wife Melinda of Winston-Salem; father of Hannah, Chris Johnson and wife Sandy of Sandy Ridge, NC; and numerous other loving family members. Funeral services for Ms. Cook will be held at 2:00 PM on Saturday, March 8, 2014 at Hayworth-Miller Silas Creek Chapel with James Yeatts and Tim Lawson officiating. Burial will follow at Gardens of Memory in Walkertown, NC. The family will receive friends from 12:30 PM until 2:00 PM on Saturday at the funeral home.
